Care Team

Our Care Team works to understand each person's situation and specific needs.  Each household completes a welcome screening with a Care Team member.  Where appropriate, referrals are provided to resources or support offered through community partners.  Assistance is also provided for those who want to sign up for benefits such as SNAP.

Free Grocery Market & Mobile Food Programs

Households who qualify can receive weekly, supplemental groceries and fresh foods through MidOhio Market at Center of Hope or by pick up at one of our mobile locations.

Specialized Food Assistance Programs

Grocery delivery and additional monthly food packs are available to seniors who meet program requirements. Please talk to a Care Team member to learn more about additional resources available.

Chaplain Services

Talk with a pastor, request prayer, ask questions, or receive support.

Health & Wellness Services

Drop in workshops, classes, and clinics.
